This model involves collaboration among mental health professionals, educators, family members, social services, and community resources to create a coordinated plan that supports the youth’s emotional, behavioral, educational, and social development. The goal is to provide holistic, continuous care that empowers the youth and their family, ensuring that services are flexible, culturally responsive, and tailored to their specific strengths and challenges.

In Home Case Management

  • Provide resources for children and families: 
    • Food Assistance, Clothing, etc
  • Assist with behavioral management:

    • Creating plans to continue education successfully
  • Coordinate with Behavioral Aide and In-Home Case Therapist to create plans for further treatment for success of youth.

Crisis Intervention

Our crisis intervention program includes an on-call rotation of qualified Treatment Coordinators who are available 24/7 to respond to emergency situations. Their primary goal is to de-escalate crises when a child becomes unruly, frustrated, or angry, ensuring the situation is managed effectively to prevent the displacement of the youth.
